What's the Deal with OTC Crypto Trading?

I keep hearing about "OTC" crypto trading for large transactions, but I don't really get how it works or who it's for. Is this just for billionaires and institutions, or can smaller investors use it too? How is it different from trading on a regular exchange like Binance or Coinbase?
